By participating with any Brookside Church event, you acknowledge and agree to the following:
Participant's Medical Condition & Risks: You confirm that you/the participant you are registering are medically able to participate in this activity. You understand that participation involves risks, which may include injury and/or, in extreme cases, death.
Financial Responsibility for Injury: You accept personal financial responsibility for any injury or loss sustained during this activity by you/the participant you are registering, as well as for any authorized medical treatment provided by event leadership.
Insurance and Emergency Costs: You understand that accident insurance coverage is limited to ordinary accident insurance, and you will be responsible for any other emergency costs incurred.
Medical Consent: You consent to any necessary medical examination, anesthesia, diagnosis, surgery, treatment, and/or hospital care for you/the participant you are registering, as advised by a licensed physician in Indiana.
Release of Liability: On behalf of yourself, your heirs, executors, and administrators, you release and forever discharge Brookside Church and its officers, employees, members, volunteers, and sponsors from any and all liability, claims, or damages arising from or connected with participation in any Brookside Church activities.
Conduct and Authority: You acknowledge that Brookside event leadership and/or league officials have the authority to suspend or dismiss participants for inappropriate conduct in a church-based, family-oriented setting. Inappropriate conduct includes, but is not limited to, foul language, belligerence, property damage, and excessive anger.
If you are registering a minor participant: By proceeding with this registration, you affirm that you are the legal guardian of the minor participant and have the authority to agree to these terms on their behalf.
